• Payment plans for extensive periodontal treatment will be considered by request.

• Payment in full by check, cash or credit card. (No 7% allowance)

Claims for initial exams, x-rays, hygiene maintenance appointments and other non-surgical procedures will be sent electronically by us to insurance companies that accept electronic submission. If your company does not accept electronic claims, we will provide a paper claim for you to mail.

For surgical or non-surgical periodontal treatments which require record data such as x- rays, periodontal readings, we will send those claims by mail from our office. In most cases we require pre-estimation of periodontal treatments to avoid duplication of office records and to inform you of what dental benefits you can expect.

We will accept assignment for dental insurance payments only if we are in-network providers on your dental insurance plan.
